During posterolateral reconstruction graft passage of the right knee with the patient in the supine position, the bone plug portions of the 2 Achilles tendon allografts are placed into the popliteal tunnel (A) and the LCL tunnel (B) in the femur and fixed using metal interference screws. The LCL graft is passed between the capsule and the iliotibial band and biceps (C) and routed through the fibular tunnel from anterolateral to posteromedial (D). (E) The knee is held at 30° of flexion, with a slight valgus force in neutral rotation, and the graft is fixated in the fibula using a bio composite interference screw. (F-G) The popliteal limb is shuttled intra-articularly and out by the proximal tibiofibular joint and the 2 free ends of the grafts are then shuttled through the tibia from posterior to anterior. (H) With the knee at 60° of flexion and neutral rotation, and slight valgus force, the graft is fixed on the tibia, using a <t>biocomposite</t> interference screw. (LCL, lateral collateral ligament.)
